Job Responsibilities
- Provide skilled nursing care to patients in accordance with physician orders, established plans of care, and applicable state and federal regulations.
- Perform nursing assessments and monitor patients for changes in condition.
- Administer medications and treatments as ordered and within the scope of practice.
- Educate patients and caregivers on disease management, medications, safety precautions, and plan-of-care instructions.
- Document all nursing visits, assessments, treatments, and communications accurately and timely in the electronic medical record.
- Communicate changes in patient condition to the supervising Registered Nurse (RN) and other members of the healthcare team.
- Collaborate with physicians, RNs, therapists, and other healthcare professionals to support positive patient outcomes.
- Follow infection control procedures, patient safety standards, and agency policies.
- Maintain compliance with all applicable Medicare, Texas Health and Human Services, and agency requirements.
- Complete assigned visits and documentation within required timeframes.
Qualifications
- Current, unrestricted Texas Licensed Vocational Nurse (LVN) license.
- Current CPR certification or willingness to obtain as required by the agency.
- Previous home health or hospice experience preferred, but not required.
- Strong communication, organization, and time-management skills.
- Ability to work independently while following established plans of care and supervisory direction.
- Reliable transportation and a valid driver's license for field-based positions.
- Ability to maintain accurate, timely clinical documentation.
