Through our Alternative Family Living (AFL) program, individuals reside in a private home setting with a family or individual who provides 24-hour care and support. This model allows residents to become part of a new family, engage in meaningful relationships, and explore opportunities such as education, employment, volunteering, and community involvement.
Position Overview
As an AFL Provider, you will welcome an adult with developmental disabilities to live in your home, offering full-time care, supervision, and support tailored to their unique needs. Providers must have a dedicated, private bedroom or living space available for the individual to ensure comfort, privacy, and dignity. You will work closely with our team to deliver high-quality, individualized care in a nurturing home environment.
Essential Responsibilities
- Provide a safe, stable, and welcoming home environment with a private room or space for the individual.
- Support daily living activities including hygiene, grooming, dressing, meal preparation, and medication administration.
- Coordinate and attend routine and emergency medical, dental, and behavioral health appointments.
- Provide transportation to appointments, community outings, and scheduled activities.
- Maintain accurate documentation including daily notes, medication records (MARs), and incident reports.
- Attend and participate in Treatment team meetings and Medication management appointments
- Ensure a backup provider is available for time off and emergencies; backup staff must complete and maintain all required training and certifications.
- Participate in recreational activities, including swimming and community events.
- Assist with household tasks such as cleaning, laundry, and minor repairs.
- Advocate for the resident and support their involvement in family and community life.
- Monitor and assist with personal care routines and adaptive equipment use.
- Maintain confidentiality and uphold the rights and dignity of the individual at all times.

Physical & Mental Requirements
- Ability to lift up to 20 lbs, stoop, bend, walk and perform household tasks.
- Adequate visual and audio acuity to supervise residents and respond to emergencies.
- Strong problem-solving, communication, interpersonal, and organizational skills.
- Ability to perform basic math functions and maintain accurate records.
Qualifications
- High School Diploma or GED required.
- Valid driver’s license with at least 3 years of driving experience and no more than 2 moving violations in the past 3 years.
- Reliable vehicle with auto insurance policy.
- Ability to care for an individual in your private home or apartment.
- Strong verbal and written communication skills.
- Must maintain all state-required certifications (CPR, First Aid, Medication Administration, etc.).
- At least one year of experience working with individuals with developmental disabilities preferred.
- Must successfully complete all required training prior to placement.
Compensation
- Annual Salary Range: $35,000 – $40,000
- This is a 1099 contracted position; providers are considered independent contractors and are responsible for their own taxes and benefits.
