AFL Services
An AFL provides residential care (24-hour services) in a home environment for individuals who have an intellectual developmental disability (IDD) or dual diagnosis of mental health/IDD. The primary purpose of these services is to provide personal care, habilitation and supervision in a residential setting through a contract with the family. The services are provided in the primary residence of a family and can be licensed or unlicensed. The Division of Health Services Regulations will license the home when the family serves either one or more minors or two or more adults who require residential services. AFL homes provide natural supports identified as places, things and, particularly, people who are a part of individual’s life. The concept is that purchased or funded supports should not take the place of natural resources available for the individual.
What is Required
Families interested in becoming an AFL Provider will complete the AFL Application on the ComServ website. An application will be completed by the family member who will provide services under contract.
Applicants will be screened and must meet paraprofessional status as defined by the State. Applicants must have a high school degree or equivalency and have two years of experience working with the population to be served. Applicants will be intereviewed to determine their ability for the job. The applicant must also provide references and pass criminal background and driving checks.
The home must also receive a safetly check by ComServ and by the responsible MCO for approval.
AFL Providers are responsible for attending and successfully completing ComServ competency based pre-service and recertification (Policy HR 010-Training and Development) as required by the AFL Agreement, prior to placement of the individual.
AFL Providers are also responsible for completing any other training specific to the individual.
AFL Providers will complete a TB test and provide supporting documentation to the Staff Development Coordinator by first day of pre-service training. The TB Questionnaire will be completed with AFL Provider annually thereafter during training recertification.
The AFL provider is not considered a staff, but the provider is paid every two weeks on a contract basis.
Monitoring of services provided in the home is the responsibility of the supervising Qualified Professional.
The monitoring will also include a vehicle check to ensure that the vehicle has a current tag.
Documentation and Billing Requirements of the Provider
The AFL provider will be responsible for completing required documentation depending on the service provided. ComServ, Inc will provide training on required documentation.
Transportation Requirements of the Provider
The contracted AFL Provider must carry $100,000-$300,000 vehicle liability coverage
and submit a copy to the QP on a yearly basis.
Submit a copy of their license and any license renewal to the QP.
Maintain their car in good working condition with tag renewed as required by DOT.
The AFL Provider is responsible for transportation to doctor appointments and any
other medical appointments.

- The home will not require a license when the family serves only one adult with a IDD/MH disability.
- The family will be responsible for obtaining a fire report from the Fire Marshall of the particular county in which the family resides. The family will be responsible for supplying ComServ, Inc. with a copy of the reports.
Additional Requirements
- AFL Providers are not employees of ComServ, Inc. and are not eligible for benefits through ComServ, Inc.
- Individuals served cannot be left in the home alone unless under the supervision of the AFL provider, back up staff or respite provider, unless the Plan states that the individual can be left alone for how long and under what circumstances.
- The guardian assumes all responsibility for transportation not provided by the AFL Provider.
- Any person found to have a criminal background is not allowed to be alone with an individual served, even with guardian permission.
- The AFL Provider is responsible for ensuring appropriate services are provided for school age children they serve.
