PROGRAM SPECIALIST

Responsible for maintaining and assuring the quality of life within an assigned home that reflects the values, goals and objectives outlined in the Exceptional Home Care’s mission statement. The Program Specialist shall be responsible for a maximum of 30 people, including people served in other types of services. The Program Specialist will provide consumer support and foster participation in home and community activities, with a focus on the development of independent and interdependent living skills.

RESPONSIBILITIES:

Coordinates and provides direct service in the following areas of activity:

  • Evaluate the work of staff/house supervisor (s) to ensure that programs are of appropriate quality and that resources are used effectively. Consumers are being active in the community and at home.
  • Establish and oversee administrative procedures to meet objectives set 6400/6100 regulations and Exceptional Home Care’s policies and procedures.
  • Rights and Privacy: Maintains confidentiality and displays respect for consumer rights and privacy.
  • Direct Care: Coordinates the provision of Role Modeling, Skill Development, Decision-Making, Activities of Daily Living, Spirituality, Training and Transportation.
  • Reviews company card receipts for grocery/personal items to ensure appropriateness.
  • Maintenance and Cleaning: Coordinates maintenance of house with Director and Executive Administrator’s advice.
  • Safety: Conducts safety drills as deemed appropriate for the home. Performs Monthly Safety Checklist duties. Provides safety orientation for staff. Performs quality assurance reviews for safety/ cleanliness of equipment and property.
  • Appointments: Ensures House Supervisor is coordinating all consumer appointments (with guardians when applicable). Attends consumer appointments when appropriate. Participates in consumer meetings: ISP, SC monitoring, SIS, etc.
  • Meetings: Meeting with House Supervisor at least monthly. Ensuring the house supervisor is holding monthly staff meetings and being involved in those meetings. Offering input and feedback.
  • Documentation: Reviews Medication Administration Records: passed/signed /current. Follows up with staff regarding medication errors. Reviews daily progress notes using critical thinking and verifies goal achievements. Completes consumer's monthly/ quarterly/ annual progress reports. Completes the HRST at least monthly. Ensures the appointment flow chart and appointment chart is updated after appointments.
  • Finances: Assists individuals with personal financial management. Manages Petty Cash and maintains log.
  • Related Duties: Performs general responsibilities required of all Exceptional Home Care employees.
  • Attendance and Punctuality: Conforms to policies on attendance and punctuality.
  • Resource Management: Manages human, physical and financial resources so as to achieve the goals of Exceptional Home Care.
  • Conduct: Conforms to Exceptional Home Care’s policies on employee conduct and appearance.
  • Other: Verifies monthly forms and records are available.
  • Accomplishes Agency’s mission and operations by completing related results/tasks as needed.
  • Assisting with the opening of new residences and new referrals.
  • If and when the need arises will fill in as house supervisor until one can be hired.
  • Completes tasks the Director and Executive Administrator assign.
  • On call rotation

QUALIFICATIONS:

Shall have one of the following groups of qualifications:

(1) A master’s degree or above from an accredited college or university and 1-year work experience working directly with individuals with an intellectual disability.

(2) A bachelor’s degree from an accredited college or university and 2 years work experience working directly with individuals with an intellectual disability.

(3) An associate’s degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with individuals with an intellectual disability.

  • Certification of Medication Manager (must meet requirement within 90 days of employment).
  • Supervisory experience preferred.
  • Must have a valid license and qualify as an insurable driver.
  • Ability to work under stressful conditions with frequent interruptions.
  • Ability to make decisions using good judgment in crisis situations.
  • Ability to work independently without supervision.
  • Ability to interpret and apply relevant policies and procedures.
  • This person will be in good health, free from communicable diseases, and able to lift and carry up to 50 pounds.
  • Has demonstrated commitment to meeting physical and spiritual needs of persons with disabilities.
  • Discrete and confidential in handling private information about self and others.
  • Demonstrates a willingness to work as a team, is well organized, exemplifies good leadership skills, is able to plan and supervise the work of others, and has strong oral and written communication skills.
  • Able to work unsupervised, coordinating direct care with indirect duties.
  • Must pass Division of Criminal Investigation and other background checks.
  • Is required to be on call.

PHYSICAL DEMANDS:

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; and taste or smell. The employee must occasionally lift or move up to 25 pounds.
