What You'll Do
- Direct and oversee all social services programs within the facility
- Lead discharge planning and coordinate community resources and post-discharge services
- Assess residents' psychosocial needs and develop individualized care plans
- Advocate for resident rights and ensure dignity, respect, and quality of life
- Provide counseling, support, and education to residents and their families
- Facilitate resident and family care conferences
- Collaborate with physicians, nursing staff, therapy, and the interdisciplinary care team
- Maintain compliance with all federal, state, and local long-term care regulations
- Develop and maintain social services policies and procedures
- Assist with surveys, audits, and regulatory compliance initiatives
- Build and maintain partnerships with community agencies and healthcare organizations
- Ensure timely and accurate documentation within the electronic medical record
- Supervise and mentor Social Services staff as needed
- Bachelor's degree required; Bachelor's or Master's Degree in Social Work (BSW/MSW) preferred
- Minimum of 2 years of social work experience in a healthcare, rehabilitation, or long-term care setting
- Prior supervisory or leadership experience preferred
- Strong knowledge of skilled nursing facility regulations, discharge planning, and resident rights
- Excellent communication, leadership, and organizational skills
- Ability to build strong relationships with residents, families, staff, and community partners
