Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and dedicated RN MDS Coordinator to join our clinical leadership team. The MDS Coordinator plays a vital role in resident care, ensuring timely and accurate completion of all MDS assessments and care plans, in compliance with federal and state regulations.
Key Responsibilities:
- Oversee the accurate and timely completion of MDS assessments.
- Ensure care plans reflect residents’ individualized needs and regulatory compliance.
- Maintain clear documentation, including reports, evaluations, and assessments.
- Lead the facility’s quality assurance initiatives related to resident care assessments.
- Collaborate with discharge planning and interdisciplinary care teams.
- Participate in state and federal facility surveys and inspections.
- Analyze QI and QM data; assist in implementing corrective actions.
- Ensure timely electronic submission of assessments.
- Communicate effectively with families, residents, and external care partners.
- Coordinate and support therapy and restorative care programs.
- Active RN license in the State of New York.
- 2 years of experience in a skilled nursing facility preferred.
- Strong knowledge of MDS 3.0 and related care planning processes.
- Excellent communication, documentation, and leadership skills.
