Salary: Up to $140,000 per year

Job Description:

Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and dedicated RN MDS Coordinator to join our clinical leadership team. The MDS Coordinator plays a vital role in resident care, ensuring timely and accurate completion of all MDS assessments and care plans, in compliance with federal and state regulations.

Key Responsibilities:

• Oversee the accurate and timely completion of MDS assessments.
• Ensure care plans reflect residents’ individualized needs and regulatory compliance.
• Maintain clear documentation, including reports, evaluations, and assessments.
• Lead the facility’s quality assurance initiatives related to resident care assessments.
• Collaborate with discharge planning and interdisciplinary care teams.
• Participate in state and federal facility surveys and inspections.
• Analyze QI and QM data; assist in implementing corrective actions.
• Ensure timely electronic submission of assessments.
• Communicate effectively with families, residents, and external care partners.
• Coordinate and support therapy and restorative care programs.
  
  
  

Requirements:

• Active RN license in the State of New York.
• 2 years of experience in a skilled nursing facility preferred.
• Strong knowledge of MDS 3.0 and related care planning processes.
• Excellent communication, documentation, and leadership skills.
• Professional, empathetic, and team-oriented approach to care.
