Key Responsibilities
- Oversee accuracy, completeness, and timeliness of all MDS assessments and supporting documentation
- Ensure compliance with state and federal regulations, including Medicare and PDPM requirements
- Conduct comprehensive resident assessments and interviews
- Develop, implement, and monitor individualized care plans for short- and long-term residents
- Lead and participate in interdisciplinary care plan meetings
- Collaborate with nursing staff, physicians, therapists, specialists, residents, and families to improve outcomes
- Provide leadership, guidance, and ongoing education to nursing staff
- Conduct in-service training on MDS, RAPS, triggers, PDPM, and Medicare guidelines
- Maintain and manage MDS and care plan scheduling for all residents
- License: Active, unencumbered Florida RN license
- Experience: Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility
- Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning processes
- Excellent leadership, organizational, and communication skills
- Ability to manage deadlines and maintain regulatory compliance in a fast-paced environment
