Key Responsibilities
- Complete and coordinate all MDS assessments in compliance with federal and state regulations.
- Develop, implement, and maintain individualized care plans based on assessment outcomes.
- Collaborate with the interdisciplinary team to ensure continuity and quality of care.
- Monitor PDPM requirements and ensure documentation supports accurate Medicare reimbursement.
- Review Quality Indicator (QI) and Quality Measure (QM) reports and implement corrective actions when necessary.
- Participate in state surveys, audits, and regulatory inspections.
- Ensure timely scheduling, completion, and electronic submission of MDS assessments.
- Educate and support clinical staff regarding MDS, PDPM, and regulatory updates.
- Maintain accurate, organized, and compliant documentation practices.
- Active Florida RN or LPN license required.
- Minimum of two (2) years of skilled nursing facility experience preferred.
- Strong knowledge of MDS 3.0, the RAI process, and care planning.
- Experience with PDPM and Medicare documentation required.
- Excellent organizational, communication, and leadership skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
