In this essential role, you will oversee and coordinate the Resident Assessment Instrument (RAI) process, ensuring accurate MDS completion, regulatory compliance, and individualized care planning that supports the highest quality of life for our residents.
Key Responsibilities
- Coordinate and oversee the completion of all MDS assessments in accordance with federal and state regulations.
- Ensure timely and accurate submission of MDS assessments and supporting documentation.
- Develop, implement, and maintain policies and procedures related to the MDS and care planning process.
- Complete medical forms, reports, evaluations, studies, and documentation as required.
- Monitor Quality Indicators (QI) and Quality Measures (QM) and implement corrective action plans when necessary.
- Coordinate interdisciplinary care plan meetings and ensure resident-centered care planning.
- Collaborate with the Discharge Planning Coordinator to facilitate effective discharge planning.
- Participate in facility surveys, audits, and government inspections.
- Establish and maintain quality assurance programs related to assessments and care planning.
- Work closely with residents, families, physicians, and interdisciplinary team members to ensure optimal outcomes.
- Support therapy services and assist with resident activity planning when appropriate.
- Evaluate resident strengths, needs, and limitations to ensure individualized care and treatment interventions.
- Current RN or LPN license in the State of Florida.
- Minimum of two (2) years of long-term care or skilled nursing experience preferred.
- Previous MDS experience strongly preferred.
- Strong knowledge of RAI/MDS regulations and reimbursement processes.
- Excellent communication, organizational, and time-management skills.
- Ability to work collaboratively with interdisciplinary teams.
