What You’ll Do
Assist with resident admissions, discharges, and transfers
Support residents and families with social, emotional, and practical needs
Help coordinate care plans and communicate with interdisciplinary teams
Maintain accurate documentation in compliance with state and federal regulations
Participate in resident assessments and care conferences
Provide support during grievance resolution and advocacy efforts
Help organize resident programs and community resource referrals
Maintain confidentiality and uphold resident rights at all times
What We’re Looking For
High school diploma or equivalent required; associate degree in human services or related field preferred
Experience in long-term care, social services, or healthcare settings a plus
Strong communication and interpersonal skills
Ability to manage multiple tasks and maintain accurate records
Familiarity with EMR systems and HIPAA regulations
