Position Summary: Carolina Senior Care, a proud provider of PACE (Program of All-Inclusive Care for the Elderly) services, is seeking a dedicated and compassionate Physical Therapist to join our dynamic team in New Bern, NC. Our mission is to enrich the lives of seniors through exceptional care, innovation, and community engagement. As a Physical Therapist, you will play a vital role in helping our participants maintain independence, dignity, and quality of life.
Essential Duties and Responsibilities:
-Conducts Assessments: Performs initial and annual physical therapy assessments for all participants, coordinating with the Interdisciplinary Team to develop and update comprehensive Plans of Care.
-Provides Treatment: Delivers physical therapy treatments, implements procedures to achieve goals, and grades activities. Confers with the Interdisciplinary Team on care issues.
-Evaluates and Recommends: Assesses participants' needs, follows up during treatment, and recommends solutions. Evaluates the need for adaptive equipment and ensures proper fit.
-Documentation and Reporting: Evaluates charts, documents progress, and maintains physical therapy notes according to charting requirements. Assists with data collection and prepares reports.
-Supervision and Training: Directs and supervises Physical Therapy Assistants, performs competencies, and provides ongoing education and clinical supervision. Trains staff on body mechanics and equipment use.
-Maintains Safety and Compliance: Ensures a safe working environment, follows policies and procedures, handles biohazard precautions, and participates in continuing education and quality improvement initiatives.
Job Requirements
Minimum Education:
Physical Therapy License in North Carolina
Minimum Requirements:
- CPR Certification (Required)
- 1 Year Working with Seniors (Preferred)
Other Requirements:
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
