Description
Summary: Provide life skills services to youth ages 16-21 who are living in a transitional living program. The youth served are either in foster care, are homeless, or at-risk of becoming homeless.
Essential Duties and Responsibilities include the following:
Demonstrate ability to be courteous, empathetic, and tactful with clients, families, referral sources, and other team members.
Keep confidential all information collected and shared regarding clients and families.
Attend and participate in all scheduled meetings.
Complete initial intake assessments with youth and repeat when needed.
Complete annual credit checks on foster youth and as needed with homeless youth.
Provide service coordination in the community (i.e. landlords, public assistance, and other service providers).
Teach daily life skills to youth within the program.
Plan and implement all independent living classes/meetings.
Coordinate with agency and community members to speak with youth during life skills classes/meetings.
Assist and counsel youth with developing educational and vocational plans.
Assist youth with completing FAFSA, ETV, college applications, course registration, etc.
Assist youth with developing resumes, filling out job applications, preparing for interviews, etc.
Complete NYTD Chafee expenditures for foster youth in the state database, FACES, on a monthly basis.
Work effectively and professionally while under pressure.
Document through case notes all direct and indirect contacts regarding youth within Agency standard timelines.
Provide referral assistance to legal services, medical services, mental health services, etc.
Oversee the Youth Board and their various activities.
Provide assistance in the event of a crisis or emergency.
Oversee IDA program.
Assist with Apartment Checks on campus.
Assist with staff shortage.
Provide on -call coverage with rotation of peers.
Maintain minimum Agency standards for continuing education and training.
Other duties as assigned.

Education and/or Experience: Bachelor’s degree from a four-year college or university in a field related to human services and preferably have two years of experience working with adolescents and young adults. Must be of good moral character and able to serve as a positive role model for youth; sometimes work under stressful situations and seek on-the-job training.
Certificates, Licenses, Registrations: Valid Driver’s License and insurable by Agency insurance company.
Computer Skills: Possess ability to use a computer and skillful with use of email, word documents, and database systems.
