Responsibilities:
- Provide emotional, spiritual, and practical support to patients and their families, addressing their social, emotional, and spiritual needs
- Conduct patient assessments, develop care plans, and collaborate with interdisciplinary teams to ensure comprehensive care
- Facilitate communication and coordination with healthcare providers, patients, and families to ensure a seamless care experience
- Identify and address social determinants of health, such as access to food, shelter, and healthcare, to promote overall well-being
- Provide bereavement support to families and caregivers, helping them navigate the grieving process
- Master's degree in Social Work (MSW)
- Active, unrestricted Wyoming LSW or LCSW license
- Strong understanding of social work theories, practices, and ethical standards
- Excellent communication, interpersonal, and counseling skills
- Ability to work independently and as part of a team
- Strong problem-solving and organizational skills
- Comfort working with diverse populations and cultures
- Per Diem position with variable hours, including weekends and holidays
- $35-45 hourly
