Job Summary:

At Everwind Hospice, we believe that compassion and care are the cornerstones of exceptional hospice care. As a Hospice Social Worker, you will play a vital role in our journey to provide personalized support and peace for patients and families in Douglas. We are seeking a highly skilled and empathetic social worker to join our dedicated team.

Responsibilities:

• Provide emotional, psychological, and social support to patients and their families, assessing their needs and developing individualized care plans to meet those needs
• Collaborate with interdisciplinary teams, including physicians, nurses, chaplains, and other healthcare professionals to ensure comprehensive care
• Develop and maintain relationships with patients, families, and caregivers, addressing social, emotional, and spiritual concerns
• Identify and address social determinants of health, such as housing, transportation, and financial needs, to improve patient outcomes
• Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making
• Provide education and support to patients and families on end-of-life care, coping mechanisms, and bereavement
• Document all interactions and assessments in accordance with agency policies and regulations
  
  
  

Requirements:

• Master's degree in Social Work (MSW)
• Active, unrestricted Wyoming LSW or LCSW license
• Strong understanding of social work theories, practices, and ethical standards
• Excellent communication, interpersonal, and counseling skills
• Ability to work independently and as part of a team
• Strong problem-solving and organizational skills
