Home Health Physical Therapist – Full Time

EverTrue Living is looking for a Full-Time Home Health Physical Therapist to join our team in the St. Louis area. This role gives you the opportunity to work one-on-one with patients in their homes, helping them maintain independence and improve quality of life.
What You’ll Do
- Evaluate patients and develop individualized treatment plans
- Provide hands-on physical therapy in the home setting
- Adjust care plans based on patient progress and needs
- Educate patients and caregivers on exercises and care plans
- Collaborate with physicians and interdisciplinary team members
- Document visits and patient progress accurately and timely
- Supervise and support Physical Therapy Assistants as needed
What We’re Looking For
- Licensed Physical Therapist (or eligible for licensure)
- Current CPR certification
- Ability to work independently and manage your schedule
- Strong communication and problem-solving skills
- Experience in home health is a plus, but not required
