Nurse Practitioner – Palliative Care

We’re looking for a Nurse Practitioner with a passion for palliative and/or hospice care to join our growing team. This is an opportunity to make a meaningful impact by supporting patients and families through complex health journeys with dignity and compassion.
What You’ll Do
- Provide comprehensive, patient-centered care to older adults in palliative or hospice settings
- Conduct assessments, diagnose conditions, and develop individualized care plans
- Collaborate with physicians and interdisciplinary teams to improve patient outcomes
- Support patients and families through serious illness, focusing on comfort and quality of life
- Prescribe and manage treatments in accordance with state guidelines and collaborative agreements
- Educate patients, caregivers, and staff on care plans and disease progression
- Coordinate care across settings and connect patients to appropriate resources
What We’re Looking For
- Active Missouri NP license (RN NP)
- Certification as an Adult or Geriatric Nurse Practitioner
- Experience in palliative care, hospice, geriatrics, or complex care management preferred
- Palliative Care certification (or willingness to obtain)
- Strong clinical judgment and ability to work independently
- Passion for working with older adults and supporting families
Salary : $115 - $135
