We are seeking a dedicated and compassionate PRN Nurse Practitioner for Palliative Care to join our dynamic healthcare team. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed history taking
- Develop and implement individualized palliative care plans aligned with patient goals and clinical best practices
- Manage complex symptoms such as pain, dyspnea, nausea, and fatigue through medication administration and non-pharmacologic interventions
- Collaborate with interdisciplinary teams including physicians, social workers, and spiritual care providers to coordinate holistic care
- Provide education to patients and families about disease progression, treatment options, and advance care planning
- Document all patient encounters accurately using EMR system in compliance with HIPAA regulations
- Participate in telehealth visits, home visits, or inpatient consultations as needed to deliver timely palliative support
Skills
- Proven experience in hospice & palliative medicine with a strong understanding of symptom management and end-of-life care principles
- Proficiency with EMR/EHR systems for efficient documentation
- Ability to work effectively with diverse populations.
- Strong communication skills for triage assessments, family education, and interdisciplinary collaboration; experience with telehealth platforms is advantageous
- Understanding of medical coding (CPT, ICD-10/ICD-9), documentation standards, ensures compliance and accurate billing
