Everlight Hospice is seeking a compassionate, organized, and detail-oriented Intake Coordinator / Administrative Assistant to join our growing hospice team. This role is essential in coordinating hospice referrals, supporting admissions, maintaining communication with referral sources and families, and assisting with daily administrative operations.
The ideal candidate is professional, calm under pressure, highly organized, and able to manage multiple priorities in a fast-paced healthcare environment while providing excellent customer service to patients, families, referral partners, and staff.
Position Summary
The Intake Coordinator / Administrative Assistant is responsible for assisting with all aspects of the patient intake and referral process, including receiving referrals, gathering intake information, coordinating communication with the clinical team, verifying insurance information, scheduling admissions, and supporting office administrative functions.
This is not a supervisory position; however, the role requires strong organizational skills, professionalism, and the ability to collaborate effectively with leadership, clinicians, referral sources, patients, and families.
Essential Job ResponsibilitiesIntake & Referral Coordination
- Receive and process hospice referrals via phone, fax, written order, or electronic communication
- Gather and document patient demographic, insurance, physician, diagnosis, and referral information
- Communicate with hospitals, skilled nursing facilities, physicians, case managers, social workers, discharge planners, patients, and families
- Assist with insurance verification and authorization processes
- Coordinate referral information with the Clinical Supervisor and admission staff
- Schedule and coordinate admission visits with clinical staff
- Maintain accurate intake logs, referral documentation, and EMR updates
- Understand hospice eligibility basics and referral workflows
- Maintain awareness of Everlight Hospice contracted facilities and referral partners
- Assist in ensuring timely and smooth transition of patients into hospice services
Administrative Support
- Answer phones and direct calls appropriately
- Provide professional customer service to referral sources, families, and visitors
- Assist with scanning, faxing, filing, data entry, and document management
- Support office organization and day-to-day administrative operations
- Assist leadership and clinical staff with administrative tasks as needed
- Coordinate office communication and scheduling support
- Maintain HIPAA compliance and confidentiality at all times
Qualifications
- Previous healthcare, hospice, home health, intake, or medical office experience preferred
- Strong administrative and organizational skills
- Excellent verbal and written communication abilities
- Ability to multitask and prioritize in a fast-paced environment
- Professional, compassionate, and dependable demeanor
- Knowledge of basic insurance verification processes preferred
- Experience with EMR systems and Microsoft Office preferred
- Ability to work independently and collaboratively as part of a team
- Hospice or home health experience strongly preferred
Schedule
- Full-time
- Monday–Friday
- Occasional after-hours coordination may be needed
