Location: Western & Central Ohio
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Hours: FT (40/week)
Weekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)
Holiday Rotation: 2 holidays per year
Summary of Position: The Palliative Care LPN delivers quality nursing care and care coordination under RN or provider direction, supporting symptom management, education, care planning, admissions, and continuity of care. The role also serves as a resource for patients, families, and referral partners regarding EverHeart Hospice’s Palliative Care services.
