We are looking for someone who can provide operational and clinical oversight and maintain our current high level of customer satisfaction. Responsibilities:

• Select personnel to supervise activities of all service areas and consult with them regarding problems.
• Represent facility with government and private agencies.
• Keeps resident’s family and physician informed of any changes in resident behavior, medications, or conditions of health.
  
• Modifies resident care plan, care directives, and activities of daily living in conjunction with resident changes in condition
  
• Train, coach, and mentor a team of nurses and caregivers.
  
• Maintain schedules and ensure proper training, disciplinary action, etc. for nurses and caregivers

Qualifications:

• Current and good standing RN or LPN license for the state of Illinois
  
• Minimum of 2-3 years of long-term care and management experience
• Clinical management experience preferred
  
• Possess strong interpersonal skills to lead employees
• Point Click Care (PCC) or other Electronic Health Record (EHR) experience
  
• Highly motivated, energetic, and dependable
• Excellent communication and customer service skills
• Be able to work with others in a team environment effectively, efficiently, and professionally
• Must enjoy the many challenges of today’s long term care field
• Demonstrated knowledge and passion for serving seniors
