Job Summary:

Palisade Healthcare Center is currently seeking a Resident Care Manager (RN or LPN) to oversee the coordination and delivery of quality resident care. The Resident Care Manager (RCM) is responsible for ensuring the proper administration of nursing care, supervising licensed nurses and nursing assistants, and working collaboratively with the interdisciplinary team to promote positive resident outcomes in our skilled nursing community.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes residents, documents significant conditions and reactions, and notifies the supervisor, physician, and resident's responsible party of changes in condition or response to medications and treatments.
• Takes and evaluates vital signs to detect deviations from normal and assess resident condition.
• Supervises Licensed Nurses and Certified Nursing Assistants to ensure compliance with facility policies and state and federal regulations.
• Assists with coaching, counseling, and corrective action when nursing staff fail to meet performance or regulatory standards.
• Updates and modifies resident care plans, care directives, Activities of Daily Living (ADL) documentation, and CNA flow sheets based on changes in resident condition.
• Collaborates with the interdisciplinary team to ensure individualized, person-centered care.
• Promotes a safe, compassionate, and positive environment for residents, families, and staff.
  
  
  

Education, Licensure, and Experience:

• Required: Graduate of an accredited school of nursing.
• Required: Current RN or LPN license in the state of South Dakota.
• Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred.
• Demonstrated knowledge of the nursing process, care planning, and regulatory compliance.
• Strong leadership, communication, and organizational skills.
