Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with the multidisciplinary team
Conduct routine assessments and monitor patient progress to ensure optimal outcomes
Collaborate with dietary and activities staff to develop and implement patient-centered care plans
Maintain accurate and up-to-date patient records and reports
Participate in quality improvement initiatives to optimize patient care and outcomes

Requirements:
Must be licensed as a Licensed Practical Nurse (LPN) in the state of Tennessee
Minimum of 0 years of experience in a long-term care or acute care setting
High school diploma or equivalent required
