- Schedule and manage care across assigned residential locations
- Maintain accurate documentation and health records
- Respond to changes in patient conditions and coordinate follow-up care
Qualifications & Requirements:
- Active New York State Registered Nurse (RN) license required
- Bachelor’s degree preferred
- Prior nursing experience required
- Experience working with developmental disabilities or community care is a plus
- Valid driver’s license and reliable transportation required
- Strong communication, organization, and care coordination skills
- Ability to work independently in a field-based setting
