Job Description:
As a Client Development Coordinator, your role will be divided equally between client recruitment and client care social work. You will be responsible for identifying and engaging potential clients while also ensuring that the needs of our existing clients are met with empathy, professionalism, and efficiency. In addition to Salary wages, Client Development Coordinators receive quarterly growth bonuses!
Schedule:
This is a full-time position, Monday–Friday, 8:30 AM–4:30 PM. Participation in the on-call rotation with the coordination team is required.
Your duties will include the following:
Responsibilities (50% Client Recruitment):
- Client Recruitment: Actively identify and engage with potential clients who require home care services.
- Sales: Present and promote our home care services to prospective clients and their families.
- Needs Assessment: Conduct thorough assessments to understand the individual needs and preferences of potential clients.
- Client Onboarding: Collaborate with Scheduling Coordinators to facilitate the onboarding process for new clients.
- Documentation: Maintain accurate records of client inquiries, assessments, and sales interactions.
- Support and Guidance: Provide information and guidance to potential clients and their families, helping them navigate our services.
- Compliance: Ensure all sales activities adhere to regulatory requirements and company policies.
- Communication: Regularly update and communicate with potential clients, keeping them informed about our services and addressing their questions and concerns.
Responsibilities (50% Client Care Social Work):
- Manage and Coordinate Client Care: Collaborate with the Scheduling Coordinator and care teams to ensure the delivery of high-quality care in clients' homes.
- Client Care Plans: Ensure client care plans are up-to-date, accurate, and reflect individual needs and preferences.
- Ongoing Support: Provide ongoing support and guidance to clients and their families, helping them navigate the healthcare system and address social and emotional needs.
- Progress Monitoring: Monitor client progress and make adjustments to care plans as needed to enhance their well-being.
- Compliance: Ensure that all care is provided in accordance with regulatory requirements, company policies, and industry best practices.
- Communication: Maintain regular communication with clients, family members, and healthcare providers to ensure everyone is informed and up-to-date on the client's care.
- Documentation: Maintain accurate and up-to-date records of client care and services provided.
Requirements:
- Degree in Social Work, Marketing, Business, Healthcare Management, or a related field (Helpful, Not Required).
- At least 2 years of experience in case management, sales, and/or client recruitment, preferably in the healthcare industry.
- Knowledge of home health care regulations and practices (helpful).
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time-management skills.
- Proficiency in Google Office (Preferred)
- Valid driver's license and reliable transportation.
