This role involves working closely with vascular surgeons, physicians, nursing staff, and other healthcare professionals to facilitate diagnostic testing, treatment, and follow-up care. The Vascular Coordinator will be responsible for providing patient education, managing scheduling, assisting with treatment planning, and ensuring that patients adhere to prescribed therapies.
- Schedule patient appointments for consultations, diagnostics, and procedures related to vascular care.
- Serve as the point of contact for patients and families, providing information and updates on their treatment plans.
- Ensure all required pre-procedure assessments, tests, and follow-ups are completed in a timely manner.
- Educate patients about vascular conditions, treatment options, and post-procedure care instructions.
- Provide information regarding lifestyle changes, medications, and therapies for managing vascular health.
- Maintain accurate and up-to-date patient records and care plans.
- Document patient interactions, treatments, and outcomes in the electronic medical record (EMR) system.
- Communicate effectively with vascular surgeons, physicians, nurses, and other healthcare team members to coordinate patient care.
- Assist in preparing and following up on patient-specific treatment plans.
- Manage outpatient clinic schedules, ensuring optimal use of resources and appointment times.
- Handle insurance verification, prior authorization, and billing processes when needed.
- Monitor patient outcomes, ensuring continuity of care and improving patient satisfaction.
- Participate in quality improvement initiatives and assist with patient surveys and feedback processes.
- Experience in vascular surgery, outpatient care coordination, or similar medical field preferred.
- Strong knowledge of vascular disease, diagnostic testing, and treatment procedures.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work in a fast-paced environment while managing multiple tasks.
- Proficient in EMR systems and Microsoft Office Suite.
