HIV Outreach Specialist: Esperanza Health Center, a primary health care clinic in Philadelphia committed to holistic, Christ-centered care, seeks a qualified and committed professional for a full-time (32 hours per week) HIV Outreach Specialist position. include ensuring patients remain in care, conducting follow-ups via phone calls, letters or home visits for missed appointments, and HIV testing and outreach in the community. The HIV Outreach Specialist must stay abreast of current trends in HIV-related research and services, especially in the health center’s service area. Other duties include building awareness in the community about the HIV services provided by Esperanza; distributing and posting flyers throughout neighborhoods in Esperanza’s service area; and establishing and maintaining relationships with agencies and programs that can refer their clients to Esperanza for care. The HIV Outreach Specialist will report to the HIV Program & Outreach Coordinator.
Essential Duties and Responsibilities:
- Strong organization and computer skills; ability to maintain and keep accurate records of outreach activities and patient data using Excel spreadsheets, CAREWare and electronic medical records.
- Ability to build and establish strong relationships with other agencies and churches within the Philadelphia community that provides services to high-risk communities.
- Ability to work independently as well as being a part of a team.
- Manage case notes and proper documentation of all testing and patient encounters.
- Ability to integrate faith and provide Christ-centered spiritual encouragement to patients in the course of care.
- Bilingual in Spanish/English required.
Education, Skills and Experience: High School diploma preferred. Previous experience conducting outreach and working with HIV/AIDS populations preferred. Knowledge of issues related to HIV, AIDS and other STIs and of community resources, churches and community organizations strongly preferred. Strong organizational skills and attention to details are essential for the position. Strong interpersonal skills, including ability to connect with patients with complex needs. Good oral communication skills.
Salary : $17 - $20
