Intensive Care Manager (ICM)

Department: Behavioral Health Services

Supervisor: MAT Supervisor

Work Hours: 40 hours per week, Monday–Friday

Location: K&A Clinic — Kensington & Allegheny

Salary Range: $49,000 – $56,000 annually (commensurate with degree, licensure, and experience)

Status: Full-Time, Exempt

REQUIRED QUALIFICATIONS

 MUST meet one of the following requirements:

  • Hold a Master's degree in Social Work, Counseling, Psychology, or a closely related behavioral health field; 

OR

  • Hold a Bachelor's degree and be currently enrolled in a qualifying Master's program in one of these disciplines.

Preferred Qualifications

  • Candidates who are working toward clinical licensure (such as LCSW, LPC, or LMFT) are strongly preferred.

JOB SUMMARY

The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health

professionals to provide comprehensive case management services to patients seeking support with substance

use. Services include referral and linkage, individualized care management, and support for adherence to

available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use

Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma-

informed approaches.

WHAT YOU WILL BE DOING

• Receive and manage patient referrals; collaborate with departmental staff to facilitate and expedite initial

screening and intake.

• Provide ongoing support and care management to individuals engaged in substance use, utilizing a

culturally sensitive, strength-based, trauma-informed approach to build and maintain a therapeutic

alliance.

• Collaborate with patients and care teams to develop individualized care management plans that address

identified needs and support progress toward patient-identified goals.

• Identify and connect patients to culturally appropriate resources across EHC and the broader community;

provide referrals to outside agencies as appropriate.

• Monitor and evaluate each patient's recovery progression; communicate progress to the care team

regularly and flag concerns as they arise.


• Address and amend care plans as needed in collaboration with patients and care teams, utilizing clinical

assessment skills to match patients with the appropriate level of care, including referrals to higher levels

of behavioral health care when indicated.

• Provide crisis intervention, crisis management, and safety planning as appropriate.

• Document all patient progress and services provided accurately and in a timely manner in the Electronic

Medical Record (EMR).


PHYSICAL DEMANDS AND WORK ENVIRONMENT

The Intensive Care Manager position involves regular in-person patient contact in a clinical outpatient setting. The

following physical demands are representative of those required to perform the essential functions of this role:

• Ability to sit, stand, and move throughout the clinic for extended periods

• Ability to use a computer and telephone for extended periods

• Occasional travel between Esperanza sites as operationally needed (less than 10%)

• Ability to remain composed and professionally effective in high-stress situations, including crisis

intervention scenarios

• Ability to lift and carry materials up to 15 lbs. as needed

YOU MAY BE A GOOD FIT IF YOU

• Hold a Master's degree in Social Work, Counseling, or a related field — OR hold a Bachelor's degree and

are actively enrolled in a qualifying Master's program. Applicants pursuing or planning to pursue licensure

as an LCSW, LPC, or LMFT are strongly preferred. Applicants who are already fully licensed (LCSW,

Psy.D, LPC, or LMFT) are encouraged to explore our Behavioral Health Consultant position.

• Are bilingual in English and Spanish (we may consider Master's-level applicants willing to participate in an

immersive language training program)

• Have recent experience providing care management to individuals in recovery or actively engaged in

substance use and other behavioral health needs

• Thrive in a fast-paced, team-oriented clinical environment

• Are energized by finding innovative ways to deliver high-quality behavioral healthcare to underserved

communities

• Want to work in a Christian healthcare ministry with colleagues who are compelled by the love of Christ to

offer care to all in need

Because this position involves direct patient contact, and because the early stages of recovery are associated

with greater vulnerability and uncertainty that could affect an individual's ability to perform the essential functions

of the role, Esperanza does require that any applicant or employee who voluntarily discloses a history of addiction

confirm that they have been in continuous recovery for a minimum of two (2) years.

This requirement applies only upon voluntary self-disclosure and is not used as a basis for disqualification outside

of the context described above.
