The Life Sharing Specialist responsibilities include planning, organizing, and directing provisions of cross-disciplinary services to clients in a community-based program for intellectually disabled individuals. Responsibilities include planning or participating in planning, developing, and evaluating therapeutic care and treatment programs. Will also act as team facilitator in Interdisciplinary Team meetings to develop Individual Support Plans (ISP’s) on each assigned client. Responsible for being on call as scheduled.
