Equity Staffing is seeking an experienced Travel Registered Nurse (RN) for a Home Health/Hospice assignment supporting patients across Portage, Summit, and Medina. This is a great opportunity for a compassionate, independent clinician who thrives in a patient-centered, field-based environment.
What You’ll Do
- Provide skilled nursing care to patients in their homes in accordance with physician orders
- Conduct patient assessments, care planning, and ongoing evaluations
- Deliver hospice and/or home health services with a focus on comfort, quality of life, and education
- Accurately document patient visits and care in HCHB (Homecare Homebase)
- Coordinate care with interdisciplinary teams including physicians, therapists, and caregivers
- Educate patients and families on disease management, medications, and care plans
- Meet productivity expectations (30 points per week)
Requirements
- Active RN license
- Home Health experience required
- Experience with HCHB (Homecare Homebase)
- Current CPR certification
- Strong clinical assessment and documentation skills
- Ability to work independently and manage a travel-based schedule
