Primary Responsibilities
- Ensure accurate, timely, and complete patient documentation
- Collaborate and communicate effectively with interdisciplinary care teams
- Assist providers and care teams with various care coordination activities
- Support benefit determination related to Medicare Part A, Part B, and Part D benefits
- Collaborate with advanced practice clinicians and facility staff to identify and respond to changes in patient condition
- Assist with therapy coordination and rehabilitation planning
- Participate in facility-based meetings and care planning discussions
- Schedule and participate in family conferences, team meetings, and case presentations
- Coordinate training initiatives and assist with quality measure compliance tracking
- Create and deliver educational in-services for staff and selected audiences
- Assist providers with documentation, coding, medication reconciliation, nursing rounds, assessments, and patient care documentation
- Write verbal orders from providers when permitted by state nursing regulations and facility practices
- Assist in managing patients experiencing acute medical conditions or exacerbations of chronic illnesses
- Support patients through short-term sub-acute rehabilitation episodes
- Maintain compliance with all clinical, regulatory, and organizational standards
- Active and unrestricted Registered Nurse (RN) license in the state of Washington or ability to obtain prior to start date
- Associate Degree in Nursing (ADN) or higher
- Current Basic Life Support (BLS) certification
- Ability to perform physical assessments and meet physical job requirements, including lifting up to 30 pounds
- Reliable transportation
- Ability to travel between multiple facilities throughout the Spokane area during the workday
- Full COVID-19 vaccination as required by client and regulatory guidelines
- Ability to successfully pass a pre-employment drug screening
- One or more years of clinical nursing experience, preferably in a long-term care, skilled nursing, senior living, or post-acute setting
- Experience caring for patients with complex medical, behavioral, social, and functional needs
- Strong understanding of geriatrics and chronic disease management
- Experience coordinating care across interdisciplinary teams
- Proficiency with electronic medical records and clinical documentation systems
