The primary purpose of this position is to coordinate social services and life enrichment activities for residents in accordance with ERS’s policies and procedures, and federal, state, and local standards, to assure that the physical, emotional and social needs of the residents are met so that they can continue living independently as long as possible.
Minimum Qualifications:
- Bachelor’s degree in social work or other related field preferred. A minimum of 4 years’ experience in a directly related field will be considered in lieu of a degree.
- Prior experience working with older adults preferred.
- Moves equipment and supplies weighing up to 25 lbs on a regular basis and up to 50 lbs on an occasional basis.
- Excellent customer service and communications skills, both written and verbal, required and ability to handle service coordination support in a professional and efficient manner. This includes the ability to read, write, and understand the English language and speak the language in an understandable manner.
- Able and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, clients, family members, volunteers, government agencies/personnel, and the general public in a fast-paced work environment.
- Must be fully up-to-date with required vaccinations.
- Provides general service coordination management including intake, tracking, and ongoing education on available services and community resources.
- Assesses residents’ needs for services and serves as a liaison with residents, family members, social agencies, and other ERS team members as appropriate to resolve social and environmental concerns.
- Supports residents by referring them to the appropriate agencies, service providers, and community resources to provide the needed services and connects them to benefits for which they are eligible.
- Maintains a service coordination resource directory which includes a listing of State and/or local service providers that residents can contact for assistance. Monitors the ongoing provision of supportive services from community agencies and networks with community providers and seeks out new services available to the residents.
- Responsible for maintaining adequate documentation on residents requiring social support services. Completes and submits reports as requested or required.
- Assists the residents in building informal support networks with other residents and broader community. Encourages residents to volunteer, lead, and participate in activities.
- Conducts research/collects data on a routine basis. Analyzes the types, frequency, and other characteristics of services that residents use, need, and want. Reports information to the appropriate management team members as requested.
- Coordinates maintenance requests for shared community spaces and maintains sufficient supplies at clubhouse.
- Arranges and coordinates for various speakers and service providers to come to the community on a regular basis to promote health, wellness, and benefits as appropriate as well as other areas of interest and/or need to the residents. Assesses residents’ participation in, and satisfaction with programs, interest in new programs, and barriers to greater participation.
- Provides regular and timely communications with residents including monthly newsletter and life enrichment calendar.
- Responsible for the management of resident clubhouse rentals including reservations and payments.
