As the QIDP, You Will:
- Provide person-centered case management by developing, implementing, and monitoring individualized service plans that promote independence, dignity, and quality of life for individuals with intellectual and developmental disabilities (IDD).
- Oversee Host Home providers and residential services by completing site visits, environmental audits, assessments, and ensuring individuals receive high-quality support that meet agency and DHS standards.
- Coordinate medical, behavioral health, psychiatric, Community Day Services, and other community-based supports to ensure each individual's needs and preferences are met.
- Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.
- Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care.
