Position Summary
The MAT (Medication-Assisted Treatment) Case Manager is responsible for providing comprehensive case management services to individuals enrolled in substance use disorder treatment programs, with a focus on those receiving MAT services. This position ensures clients receive coordinated, person-centered care that addresses medical, behavioral, and social service needs in compliance with Virginia Medicaid and DBHDS regulations.
Key Responsibilities
- Conduct intake assessments, gather psychosocial history, and develop individualized service/care plans.
- Coordinate MAT services with prescribing providers, counselors, and external healthcare professionals.
- Monitor client progress and update treatment/service plans as needed.
- Provide referrals and linkage to community resources (housing, employment, social services, primary care).
- Ensure compliance with all Virginia DMAS and DBHDS standards for documentation, billing, and service delivery.
- Educate clients about MAT, recovery supports, harm reduction, and relapse prevention.
- Advocate for client needs while supporting autonomy and recovery goals.
- Facilitate care coordination across interdisciplinary teams and participate in case conferences.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
- Assist clients in navigating insurance/Medicaid benefits, including prior authorizations and renewals.
- Participate in staff meetings, trainings, and quality assurance reviews.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field (required).
- Current QMHP-A, QMHP-T, or CSAC certification/licensure in Virginia (preferred).
- At least 1 year of experience providing case management, preferably in substance use disorder or behavioral health settings.
- Knowledge of MAT medications (e.g., buprenorphine, methadone, naltrexone) and substance use treatment modalities.
- Familiarity with Virginia Medicaid guidelines and DBHDS regulatory standards.
- Strong organizational, communication, and advocacy skills.
- Ability to work with diverse populations with sensitivity and professionalism.
- Valid Virginia driver’s license and reliable transportation (for community-based services).
Working Conditions
- Primarily office-based with frequent community outreach and client engagement in clinical and non-clinical settings.
- Flexible scheduling may be required to meet client needs.
Physical Requirements
- Ability to sit, stand, and walk for extended periods.
- Occasional lifting (up to 20 lbs.) for files, supplies, or outreach materials.
