Case Manager

Description

Duties and Responsibilities: 

1. Initiate, conduct, assign and follow up on all investigations of client abuse, neglect, and/or exploitation
2. Promotes the company at conferences, provider fairs, and other venues
3. Monitor overall client care and progress
4. Participate in treatment team meeting to determine appropriate levels of care
5. Conduct, screenings, admissions, referrals, and assessments
6. Approve and monitor Client Service Plans
7. Write annual and quarterly reports
8. Perform shift work during staffing shortages
9. Develop ISP’s
10. Transport and/or link clients to community appointments as needed
11. Participate in monthly on-call rotation
12. Complete signature and obtain signatures on documents, as needed
13. Provide Case Management services of linking, coordinating, discharge planning for individuals to community resources, as needed. 
14. Assist with facilitating transportation of client to a from facility and community-based services.
15. Submit all clinical documentation to include, progress notes, treatment plans, quarterly reports, assessments within 24 hours, as required by Virginia State Medicaid guidelines.
16. Assist with home visits
17. Other duties as assigned

Knowledge, Skills, and Abilities

1. Compliance with all applicable laws and regulations regarding company policies and procedures, Labor Laws, the VDBHDS, Office of Human Rights, and OSHA
2. Fiscal operations, risk and personnel management and assessment tools
3. Records Management and confidentiality practices
4. Conducting and reporting abuse and /or neglect investigations
5. Conducting Quality Assurance Check on all documentation generated by the site staff and risk management factors
6. Monitor overall client care

Qualifications

Qualifications:

1. Minimum Associates degree in human services Field with a minimum of one (1) year of work experience in working with persons with a Mental Illness, Substance Use Diagnosis or developmental disability, or 1 year experience in Addiction and Recovery with CSAC-A credential
2. LPC, LPC-Eligible, LCSW, LCSW-Eligible, CSAC, CSAC-S, CSAC-A
3. Certifications are required in CPR, First AID, and Behavior Modification
4. Must submit to a national and state background investigation and a central registry check by the Department of Social Services
5. Have a valid Virginia driver’s license and current DMV driving record
6. Must have the ability to lift up to 50 pounds and be able to stoop, bend, and stand.
