Description
Duties and Responsibilities:
- Initiate, conduct, assign and follow up on all investigations of client abuse, neglect, and/or exploitation
- Promotes the company at conferences, provider fairs, and other venues
- Monitor overall client care and progress
- Participate in treatment team meeting to determine appropriate levels of care
- Conduct, screenings, admissions, referrals, and assessments
- Approve and monitor Client Service Plans
- Write annual and quarterly reports
- Perform shift work during staffing shortages
- Develop ISP’s
- Transport and/or link clients to community appointments as needed
- Participate in monthly on-call rotation
- Complete signature and obtain signatures on documents, as needed
- Provide Case Management services of linking, coordinating, discharge planning for individuals to community resources, as needed.
- Assist with facilitating transportation of client to a from facility and community-based services.
- Submit all clinical documentation to include, progress notes, treatment plans, quarterly reports, assessments within 24 hours, as required by Virginia State Medicaid guidelines.
- Assist with home visits
- Other duties as assigned
Knowledge, Skills, and Abilities
- Compliance with all applicable laws and regulations regarding company policies and procedures, Labor Laws, the VDBHDS, Office of Human Rights, and OSHA
- Fiscal operations, risk and personnel management and assessment tools
- Records Management and confidentiality practices
- Conducting and reporting abuse and /or neglect investigations
- Conducting Quality Assurance Check on all documentation generated by the site staff and risk management factors
- Monitor overall client care
Qualifications
Qualifications:
- Minimum Associates degree in human services Field with a minimum of one (1) year of work experience in working with persons with a Mental Illness, Substance Use Diagnosis or developmental disability, or 1 year experience in Addiction and Recovery with CSAC-A credential
- LPC, LPC-Eligible, LCSW, LCSW-Eligible, CSAC, CSAC-S, CSAC-A
- Certifications are required in CPR, First AID, and Behavior Modification
- Must submit to a national and state background investigation and a central registry check by the Department of Social Services
- Have a valid Virginia driver’s license and current DMV driving record
- Must have the ability to lift up to 50 pounds and be able to stoop, bend, and stand.
- Basic computer operations
- PPD Screening
