Requirements
You will serve as the Registered Nurse Supervisor and Administrator of our Wisconsin PCA program — carrying dual regulatory accountability from day one. The role begins part-time during certification and converts to full-time upon approval, with responsibility for building and overseeing our Wisconsin clinical operations and eventually leading a team of RN Supervisors across our service area.
Phase 1: Certification (Part-Time, ~5 hrs/week)
Serve as RN Supervisor and Administrator of Record on the DHS 105.17 application
Review, develop, and attest to clinical policies and procedures required for certification
Advise on Wisconsin PCA regulatory standards (DHS 105.17, DHS 107.112, N 6.02/6.03)
Phase 2: Launch (Full-Time)
Conduct initial client assessments and develop individualized care plans per DHS 107.112(3)(a)
Secure and manage physician orders; coordinate renewals per DHS 107.112(3)(a)2.
Conduct 60-day supervisory home visits with direct observation of personal care workers per DHS 107.112(3)(c)
Train and evaluate personal care worker competency prior to independent service delivery per DHS 105.17(1n)(a)(2)
Develop and manage time and service reporting systems for care staff per DHS 107.112(3)(a)4.
Monitor member health status; coordinate with physicians on significant changes per DHS 107.112(3)(a)3m.
Oversee agency infection control program per DHS 105.17(1r) and ch. DHS 145
Maintain clinical records and ensure full documentation and EVV compliance
Phase 3: Scale (Full-Time) As our Wisconsin program grows, your role evolves from direct clinical oversight to leading a team of RN Supervisors — supporting hiring, training, and managing clinical staff across our service area.
What You'll Bring
Baseline Requirements
Active RN license in Wisconsin
Direct care experience with elders or adults with disabilities
Valid driver's license, reliable transportation, and willingness to travel to client homes
Comfortable operating in an early-stage environment
We are looking for someone who can function as both an RN and a Supervisor. Experience in all of the following areas is highly preferred.
Healthcare Delivery
Healthcare supervisory or administrative experience
Patient assessment, staff evaluation, competency determination, and referral experience
Obtaining and managing written physician orders
Developing and managing patient plans of care and instructing care staff
Staff training, competency assessment, and performance evaluation
Developing and managing time reporting systems for care staff
Creating or managing mentoring or coaching programs
Conducting unannounced home visits to monitor care delivered by healthcare workers
Healthcare Supervision
Training and experience in health care administration
Supervisory or administrative experience in health care or personal care, including oversight of supervisors, RNs, and healthcare aides
Experience following and applying Wisconsin Administrative Rules — please be able to name the specific rules
Experience training, evaluating competency, and supervising employees
Experience creating healthcare-related policies and procedures
Experience overseeing and managing agency finances
Experience overseeing and managing an agency infection control program
Salary : $55 - $60
