What are the responsibilities and job description for the Registered Nurse position at Cornerstone RPO?
The Registered Nurse – Case Manager (Hospice) plans, organizes, directs, and coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The incumbent builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Location: Milwaukee, Wisconsin and Waukesha, Wisconsin
Work Arrangement
· Participate in holiday rotation – about 2 holidays per year. No regular on-call or weekend coverage for RNCM unless there are extenuating circumstances.
Responsibilities:
· Update and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s)
· Initiate proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care
· Provide skilled nursing care and provide patient, caregiver, and family counseling
· Communicate with primary physician and other healthcare professional when necessary and at least every 60 days
· Obtain all necessary physician orders and collaborate with physician as patient’s needs dictate
· Prepare clinical notes; 75% charting to be completed in the patient’s home
· Re-evaluate the need for continued care on an ongoing basis, initiate recertification as necessary
· Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient
· Supervise Licensed Vocational Nurses (LVNs), Licensed Practical Nurse (LPNs), Home Health Aide (HHAs), and Certified Nursing Assistants (CNAs)
Education and Experience
· Requires graduation from an approved School of Nursing
· Requires an active, unrestricted license for the state of Wisconsin
· Experience in public health and/or home care nursing is preferred but not required
· Management experience not required
Licensure
· Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
Skills
· Self-directed and able to work with minimal supervision
· Demonstrate excellent observation, problem solving, communication (listening, verbal and written), and nursing skills per competency checklist
· Ability to organize and prioritize workload independently
· Proficiency with Microsoft Office Suite
