Position Overview:

We are seeking a detail oriented and organized Care Manager to conduct home visits, train, and support our caregivers, ensuring they can meet 

their loved ones’ needs.In this role, you will monitor and report on clients’ health, support personalized care plans, ensure ongoing quality care, and maintain compliance with all relevant regulations and standards. Your focus on excellence and compassion will 

contribute to creating an exceptional experience for caregivers and their loves ones.



What You Will Bring:

  • Bachelor's degree required, must be LSW or LISW, human services, social work, or a related field.

  • At least one year of experience working with elders or adults with disabilities.

  • Valid driver's license and automobile insurance, as travel within Ohio is required.

  • Demonstrated experience in case management, care planning, and assessment in a healthcare setting.

  • Outstanding proficiency in common software and web-based applications including Microsoft Excel, Word, PowerPoint, and Teams.

  • Master's degree in social work or a related field preferred.

  • Experience working in Structure Family Caregiving (SFC) or a similar setting preferred.

  • Certifications in gerontology or case management preferred.

  • Familiar with using HubSpot preferred.

Key Responsibilities:

  • Home visits: Conduct home visits to assess and create personalized care plans for clients and their caregivers.

  • Training & Support: Train and support caregivers, ensuring they can meet their loved ones' needs.

  • Reporting & Coordination: Monitor and report on clients’ health, coordinating with their primary care physicians, community resources.

  • Ongoing care: Facilitate smooth transitions and ongoing care for clients.

  • Compliance: Maintain compliance with all relevant regulations and standards.

  • Community awareness: Participate in community events to spread awareness and enroll new caregivers.

  • Role Model: Embody Entyre Care’s mission, values, and culture, acting as a role model for the team.
