Position Summary
The Housing Coordinator secures and sustains safe, recovery‑supportive housing for clients with substance use disorders. This role manages bed/room assignments in program housing, coordinates placements with community landlords and housing programs, supports residents’ stabilization plans, and ensures compliance with confidentiality and housing standards. The Coordinator is a visible, hands‑on presence—comfortable in the field, skilled at problem‑solving, and committed to harm reduction and/or abstinence‑based approaches consistent with program policy.
Key Responsibilities1) Admissions, Bed Management & Onboarding
- Manage referrals, waitlists, and eligibility screenings; maintain an accurate “bed board” and target occupancy (e.g., ≥95%).
- Coordinate move‑ins/move‑outs, complete housing assessments, obtain releases of information (42 CFR Part 2/HIPAA), and orient residents to house rules, safety procedures, and community expectations.
- Develop initial Housing Stabilization Plans within the first week of entry and update at key milestones or level‑of‑care changes.
2) Resident Support & Care Coordination
- Conduct regular check‑ins with residents; identify barriers (e.g., ID, benefits, credit, legal) and connect to internal and external services (employment, benefits, medical, MAT, mental health).
- Collaborate closely with clinical teams (counselors, peer specialists, case managers) to align housing goals with treatment/recovery plans; support transitions to higher/lower levels of care as needed.
- Use motivational interviewing, de‑escalation, and strengths‑based approaches; document progress and incidents within required timeframes.
3) Community Housing Placement & Landlord Relations
- Build and maintain a network of property owners, managers, shelters, and Continuum of Care partners; accompany clients to showings and help negotiate reasonable lease terms.
- Coordinate applications, deposits, rental assistance, and move‑in logistics (e.g., Rapid Re‑Housing, PSH, HCV, VASH, HOPWA where applicable).
- Educate clients on tenant rights/responsibilities; mediate landlord/tenant issues and support reasonable accommodation requests.
4) Property Operations & Safety (Program Housing)
- Conduct room/house inspections and ensure habitability, cleanliness, and maintenance follow‑up; manage keys, visitor logs, curfews, and other community standards.
- Implement emergency and relapse response protocols, including coordination after incidents, overdoses, or safety concerns per policy; maintain naloxone readiness and incident reports.
- Enforce rules consistently and respectfully; apply a restorative, fair‑process approach to corrective actions.
5) Data, Compliance & Reporting
- Enter services, contacts, and outcomes into the EHR/HMIS with high data quality (timely, accurate, complete).
- Track and report key indicators: occupancy, length of stay, time from referral to placement, exits to permanent housing, returns to homelessness, critical incidents, and client satisfaction.
- Protect client privacy and confidentiality at all times (42 CFR Part 2, HIPAA); follow organizational policies and local/state recovery housing standards (e.g., NARR‑aligned, where applicable).
6) Partnerships & Systems Navigation
- Participate in Coordinated Entry and case conferencing meetings; liaise with hospitals, detox/residential programs, courts, probation/parole, and community providers.
- Advocate for clients’ housing access and reasonable accommodations; troubleshoot barriers related to criminal history, credit, or documentation.
Qualifications
Required
- High school diploma/GED and 2 years relevant experience (housing, case management, shelter/homeless services, or SUD/behavioral health).
- Working knowledge of basic landlord‑tenant principles, Fair Housing concepts, and habitability standards.
- Strong documentation and time‑management skills; proficiency with HMIS/EHR and Microsoft/Google productivity tools.
- Valid driver’s license, insurable driving record, and reliable transportation for field work.
- Ability to pass background checks and meet health/safety requirements (e.g., TB test, vaccinations per policy).
- Commitment to trauma‑informed, culturally responsive care and non‑stigmatizing language.
Preferred
- Lived experience of recovery (people in recovery strongly encouraged to apply).
- Certifications: Certified Peer Recovery Specialist (CPRS), Community Health Worker (CHW), CPR/First Aid, naloxone training.
- Familiarity with HUD‑funded programs (Rapid Re‑Housing, PSH, HCV), Coordinated Entry, and Continuum of Care processes.
- Knowledge of ASAM levels of care and how housing supports treatment/recovery pathways.
- Bilingual or multilingual ability.
