Duties
- Assume leadership responsibilities by supervising nursing staff, delegating tasks, and ensuring high standards of patient care
- Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history reviews
- Administer medications and treatments accurately, including injections, IV infusions, catheterizations, and tube feedings in accordance with physician orders
- Coordinate multidisciplinary care plans involving case management, pain management, dementia care (including Alzheimer’s and memory care), and hospice services
- Maintain meticulous medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations
- Oversee infection control protocols, sterile processing procedures, and aseptic techniques to promote patient safety
- Provide triage assessments for urgent needs, utilizing emergency medicine skills such as airway management and first aid while collaborating with hospital or Level I/II trauma center teams when necessary
Qualifications
- Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), or Pediatric Advanced Life Support (PALS) are highly desirable
- Demonstrated experience working with geriatric patients in settings like skilled nursing facilities, assisted living communities, or hospitals
- Strong knowledge of ICD-9/ICD-10 coding systems for accurate medical documentation and billing processes
- Prior experience in critical care environments such as ICU or emergency room settings preferred; familiarity with Level I/II trauma centers is a plus
- Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms for efficient documentation and communication
- Expertise in specialized areas such as Alzheimer’s/dementia care, pain management (including acute pain), infectious disease care, ventilator management, and tube feeding techniques
- Excellent caregiver communication skills with the ability to educate patients and families effectively while maintaining clinical confidentiality policies
- Knowledge of long-term care regulations and familiarity working with individuals with disabilities or developmental challenges is advantageous
