Licensed Vocational Nurse - Charge Nurse

Duties

• Provide direct patient care with a focus on assessment, medication administration, wound care, and vital signs monitoring.
• Supervise and support nursing staff, ensuring adherence to clinical protocols and quality standards.
• Coordinate interdisciplinary communication for effective discharge planning and case management.
• Oversee medication management including IV infusion, injections, and tube feeding procedures while maintaining strict compliance with HIPAA and confidentiality policies.
• Conduct comprehensive patient assessments, including physical examinations and diagnostic evaluations such as laboratory specimen collection and processing.
• Manage electronic health records (PCC systems) documentation accurately using platforms like Epic or Cerner.
• Ensure compliance with long-term care regulations and safety protocols specific to skilled nursing facilities or assisted living environments.

Qualifications

• Valid LVN license with current state licensure required.
• Demonstrated expertise working with geriatric patients, individuals with developmental disabilities or memory impairments like Alzheimer’s disease.
• Strong knowledge of medical terminology, physiology, pharmacology related to pain management and infection control practices.
• Excellent communication skills with the ability to lead a team effectively while maintaining clinical confidentiality policies.
• Experience working within long-term care regulations or sub-acute care settings; knowledge of hospice or home & community care is a plus.

Pay: $30.00 - $33.00 per hour
