Duties
- Provide comprehensive nursing care to patients.
- Conduct patient assessments by monitoring vital signs, performing physical examinations, and evaluating symptoms to inform treatment plans
- Administer medications and injections accurately following physician orders while adhering to strict safety protocols and HIPAA confidentiality policies
- Document all patient interactions thoroughly in electronic health records (PCC)
- Collaborate with interdisciplinary teams including physicians, case managers, and therapists to coordinate patient care and discharge planning
Qualifications
- Valid LVN license in good standing with the state licensing board
- Strong background working with geriatric patients, dementia or Alzheimer’s care, memory care, and individuals with developmental disabilities
- Excellent communication skills combined with a compassionate approach to patient-centered care
Pay: $30.00 - $33.00 per hour
