Duties
- Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans.
- Implement evidence-based physiotherapy techniques, including trigger point therapy and orthopedics interventions, to facilitate recovery.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between inpatient, outpatient, and home health care.
- Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes.
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies within home and community care settings.
- Document all patient interactions accurately using electronic medical record (EMR) systems while maintaining confidentiality.
- Provide specialized care for diverse populations including pediatrics, geriatrics, and individuals recovering from orthopedic or neurological conditions.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience in acute care, hospital environments, outpatient clinics, or home health services.
- Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice.
- Experience developing comprehensive care plans that incorporate discharge planning and patient-centered goals.
- Familiarity with EMR systems for documentation and patient record management.
- Expertise in various physiotherapy techniques such as trigger point therapy and orthopedics treatments.
