.We are seeking a passionate and dedicated full-time Physical Therapist Assistant (PTA) to join our growing home health team. In this role, you will work under the direction and supervision of a licensed Physical Therapist to implement customized rehabilitation plans. You will travel to patients' homes to deliver hands-on treatment, help them regain mobility, manage pain, and restore their independence.Key Responsibilities:
- Deliver high-quality physical therapy interventions and treatments according to the established plan of care.
- Monitor, document, and report patient progress, responses to treatment, and any changes in status to the supervising Physical Therapist.
- Educate patients, families, and caregivers on safe mobility techniques, home exercise programs, and assistive device utilization.
- Collaborate closely with an interdisciplinary team—including PTs, nurses, and occupational therapists—to ensure coordinated, holistic care.
- Maintain accurate, timely electronic documentation in compliance with agency, state, and federal regulations.
- Follow all safety, infection control, and professional practice standards on the rehabilitation floor and within the home setting.
Qualifications:
- Graduate of an accredited Physical Therapist Assistant (PTA) program.
- Current, unrestricted state licensure or eligibility for licensure as a Physical Therapist Assistant.
- Prior experience in home health care or geriatrics is preferred, but motivated clinicians looking to transition to home care are welcome to apply.
- Active CPR/BLS certification.
- Valid driver’s license, reliable transportation, and automotive insurance.
- Strong communication, interpersonal, and time-management skills.
