Job Summary

The Case Coordinator is responsible for incorporating several professional functions into the management of the total episode of care for patients on their assigned unit(s). She/he uses independent judgment in connection with her/his managerial responsibilities.

**Job Essentials: The ideal candidate will meet the following requirements and competencies.**

• Visiting Nurse or Case Management experience preferred or 5 years' acute care Nursing experience required.
  
  

Education Requirements

• BSN
• License/Certification/Registry:** NJRN BLS required.
