At Encompass Community Supports, our full-time employees work 37.5 hours weekly.
Our LPN – Outpatient Behavioral Health provides professional nursing care in an outpatient treatment and recovery clinic. Responsibilities include direct client care, medical counseling, education, and consultation. Supports efficient clinic operations by assigning daily tasks, monitoring workflow, and reporting concerns to the Medical Supervisor. Ensures quality of care, accurate client records, and a professional clinic environment. Collaborate with team members and outside providers to support continuity of care. Works under the general supervision of the Medical Supervisor.
SOME OF THE ESSENTIAL FUNCTIONS:
- Apply the nursing process to assess, plan, implement, and evaluate care for clients.
- Guide and support daily clinic workflow.
- Follows orders directed by the agency prescribers for outpatient clinics.
- Provide direct client care including monitoring, screening, assessment, and professional communication to prevent harm and ensure safety.
- Track and complete documents, assessments and orders in compliance with governing agencies.
- Coordinate care with internal and external stakeholders to support continuity and quality of services.
- Ensure compliance with HIPAA, documentation standards, data accuracy, and agency/regulatory policies.
- Participate in quality improvement through audits, case reviews, and team development activities.
- Maintain a clean, safe clinic environment; manage medication handling, infection control, and crisis response.
- Organize and maintain accurate medical records, reports, and tracking systems in alignment with agency protocols.
- Meet training, licensure, supervision, and administrative requirements.
- Support fiscal accountability through timely documentation and utilizing agency time effectively.
- Assess and respond to behavioral health crises, including management of suicidal risk.
- Accurately measures/manages/reports client monitoring data/information and assessments in compliance with clinic processes.
- Administer injections safely and confidently as part of routine care.
- Administers TB screening as requested
- Responds to medical/behavioral matters within clinic environment
- Covers for other sites as needed
- Attends training and maintains certifications as required by the Agency and regulatory guidelines.
- Other duties as assigned
REQUIRED QUALIFICATIONS:
- State Endorsed Practical Nursing Certificate;
- Must be a licensed with the Department of Health Professionals – Board of Nursing as a Licensed Practical Nurse allowed to practice in the Commonwealth of Virginia.
- Preferably 3 years of experience in nursing, substance disorders, and/or mental health.
- Comply with Board of Nursing requirements to maintain licensure as a Licensed Practical Nurse.
- Ability to lift materials up to 25 pounds.
- Valid Virginia Driver’s License
