What are the responsibilities and job description for the Nurse Practitioner position at Empowered Staffing?
Empowered Staffing is partnered with a mobile wound care company that provides care to individuals in skilled nursing facilities. As they continue to grow, they are seeking a Nurse Practitioner to join their team in the Southern Illinois market.
Responsibilities:
- Perform comprehensive physical exams and wound assessments for patients with acute and chronic conditions
- Evaluate wound-related conditions and comorbidities to support optimal healing outcomes
- Perform wound debridement and dressing changes in accordance with clinical best practices
- Develop and implement individualized care plans in collaboration with the Medical Director and care team
- Accurately document patient encounters, procedures, and communications in the EMR within 24 hours
- Educate patients and caregivers on wound care, prevention strategies, and chronic condition management
- Communicate effectively with primary care providers, specialists, and interdisciplinary teams
- Collaborate with clinical staff and leadership to maintain efficient workflows
- Participate in clinical meetings, training, and quality improvement initiatives
- Advocate for patients by ensuring access to appropriate care and follow-up services
- Maintain compliance with clinical, regulatory, and safety standards
- Stay current with CMS guidelines, documentation requirements, and best practices
- Maintain licensure and pursue ongoing education in wound care and related specialties
- Perform additional duties as assigned
Qualifications:
- Active, unrestricted Nurse Practitioner license in Illinois.
- Clinical experience in wound care or a related specialty preferred.
- Strong skills in debridement and care planning.
- Excellent communication and documentation skills.
- Ability to work independently and collaboratively within a clinical team
