Care Coordinator –

Overview

Empower Nation is seeking a dedicated and organized Care Coordinator to join our team in the Greater Philadelphia Area. This role is essential in ensuring that our clients receive the highest level of care and support. The Care Coordinator will be responsible for managing client care plans, coordinating services, and acting as a liaison between clients and healthcare providers. The ideal candidate will possess strong communication skills, attention to detail, and a passion for helping others.

What You'll Do Daily

• Visit assigned senior living communities across the Greater Philadelphia area (daily travel required).
• Engage residents with empathy and professionalism, helping identify those who may benefit from Empower Nation's mental health or specialty care services.
• Coordinate patient scheduling, intake, and follow-up through our secure electronic health record (EHR) system.
• Serve as a trusted liaison between residents, facility staff, and Empower Nation's clinicians.
• Maintain accurate documentation and manage communications via iPad-based applications.
• Work toward monthly financial incentives tied to the success and impact of your work.

• Sales-minded attitude — comfortable initiating conversations and motivating others to engage in care.
• Reliable transportation (personal vehicle required) to travel between multiple senior facilities daily.
• Tech-savvy and comfortable using iPads, EHR systems, and mobile applications.
• Hard-working and self-motivated individual who thrives in a performance-based environment and wants to be rewarded for impact.

Job Types: Full-time, Part-time

Pay: $ $43.00 per hour

Expected hours: 20 – 40 per week
