- Identify individual social and emotional needs of residents
- Develop and maintain individualized care plans
- Participate in the planning of resident admission, community re-entry, or transfer to another facility
- Participate in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of residents
- Previous experience working in long-term care preferred
- Bachelor's degree in Sociology preferred
