MDS Coordinator Summary
As our MDS Coordinator, you will oversee all aspects of the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) process. You’ll ensure assessments are completed accurately and in compliance with CMS and Long-Term Care Insurance (LTCI) regulations and guidelines.

Key Responsibilities
- Collaborate with nursing staff and interdisciplinary team members to ensure accurate care plan goals.
- Communicate effectively across departments to ensure timely completion and transmission of RAI/MDS.
- Prepare accurate reports based on RAI/MDS data, including Quality Measures (QM) and Quality Indicators (QI).
