- Plan, implement, and oversee a comprehensive social services program that provides counseling and support for residents and their families.
- Assess residents’ medical-related social needs and develop appropriate care and treatment plans.
- Coordinate and lead family meetings to foster effective communication between residents, families, and the care team.
- Serve as a liaison to uphold residents’ rights and ensure quality communication across all parties.
- Manage and coordinate discharge planning to ensure smooth transitions of care.
- Promote a compassionate, resident-centered environment that respects the dignity and individuality of every resident.
- Bachelor’s degree or higher in Social Work (required).
- Previous experience in a Nursing Home or Long-Term Care (LTC) setting is required.
- Full-time schedule (Monday-Friday).
